Summary

These Explanatory Notes (ENs) contain guidance primarily for consultant ophthalmologists and hospital eye clinic staff in England about who should be certified as sight impaired and severely sight impaired. The ENs provide guidance on how to complete the Certificate of Vision Impairment (CVI) form and its use in the certifying process, and have been updated in tandem with a revision of the CVI form itself. The ENs have been developed in partnership with The Royal College of Ophthalmologists, the
Royal National Institute of Blind People, the Certifications Office at Moorfields Eye Hospital, the Association of Directors of Adult Social Services, NHS England, the Department for Work and Pensions and with advice from the Department for Health and Social Services in the Welsh Government, the Health and Social Care Board in Northern Ireland and the Scottish Government, and have been the subject of a public consultation.
